I would like you to take over the administration of my plan. What do you need?

The following is a list of items we will need in order to take over your plan:

• Plan Adoption Agreement with any amendments
• Summary Plan Description, if available
• IRS Favorable Determination Letter, if available
• Form 5500 and Attachments for the last 2 Years
• Actuarial Valuations for the last 2 Years
• Year-end Statement of Plan Assets for the last 2 Years
• Compensation History for ALL employees for the last 2 years
• Current Employee Census

We may need a few more items, but this is a good start.
